I, Sam Gardner, of Preventive Medicine, hereby acknowledge my request for Union Street Media to set up a MailChimp account for our company, Preventive Medicine, on May 7, 2014.
With this sign off, I confirm my understanding that the request I have made (described above) is estimated to take between 1-2 hours, is out of scope of my initial project budget, and may result in an overage of $125-$250. The overage will also include an initial software set up fee of $375. In other words, the overage will result in a $500-$625.
I understand that my approval of this overage may result in my having to pay the above-mentioned cost above the high end of my initial contract budget, will not be included in the barter originally discussed with Ted, and extend the timeline of my project.
