I, Kent Whitworth, of ‘Ohana Pest Control, hereby accept and approve the logo designs http://bit.ly/1tklvXQ, as presented to me by Union Street Media, today {10/2/14}, as the logo that will be used for my site and business.
With this sign off, I fully understand that that any revisions I request to the logo after this sign off may result in incremental costs above the high end of my initial contract budget of $3,500 to $4,250 and extend the timeline of my project.
