I, Jennifer White, of Workroom Supply LLC, hereby acknowledge my request for branding design services on March 15, 2023.
I understand that the described request(s) is/are outside the scope of the original contract between Scout Digital and Workroom Supply LLC, and is/are estimated to take 6 hours, which will be billed at Scout Digital’s support rate of $200/hour for a total of $1,200.
I understand my obligation to pay the above-mentioned cost due upon receipt of the invoice.
Note: This change order does not take into account future requests for revisions and iterations. In the event further revisions are requested, this will require another change order form.
With my signature, I accept all of the terms outlined above.
