With this sign off, I permit USM Interactive to launch the new form integration.
With this sign off, I confirm my understanding that additional revisions requests I request to USM Interactive will by routed through and billed accordingly through the Support team at my hourly rate of $150/hr.
With this sign off form, I confirm my understanding that failure to pay my monthly fees or final project fees covering the cost up to the low end, between the low and high end, and any additional overages approved during the project process, may result in my site being taken down until said payments are received by USM Interactive.
Note: USM Interactive reserves the right to delay a launch at their discretion. Launch sign off forms must be received no later than 12:00 noon EST the day prior to a launch.
