I, Anne Adler, of Fibrolamellar Cancer Foundation, hereby acknowledge my request for pre-launch site revisions, on March 29, 2016. The revisions include the following:
- Footer navigation design: 4 -5 hours
- Footer navigation development: 6 – 8 hours
- Doctor search: 10 – 14 hours
- Search by zip code and city
- Filter by specialty
- Interactive map
With this sign off, I confirm my understanding that the request I have made (described above) is estimated to take between 20 - 27 hours, is out of scope of my initial project budget.
I understand that my approval of this overage may result in my having to pay the above-mentioned cost above the high end of my initial contract budget of $2500- $3375 and extend the timeline of my project.
